About GDMT.org
GDMT.org is an educational tool for clinicians caring for patients with heart failure. It shows, for a given patient, which foundational (FMT), additional (AMT) and interventional (GDIT) therapies the 2026 ESC guideline supports, and how kidney function, potassium and haemodynamics change what can be started, continued, uptitrated, held or stopped.
The evidence matrix records, stage by stage, what each FDA label, guideline and authoritative review says, in its own words, with the page or section cited. When the sources disagree, the tool follows the FDA label first, then the guideline, then reviews, and shows every competing statement so you can judge for yourself.
Classification
Only two phenotypes are used: HFrEF (LVEF below 50%) and HFpEF (LVEF 50% or above), following the 2026 ESC guideline and the 2026 Second Universal Definition of Heart Failure. The former HFmrEF band (41–49%) is treated as HFrEF in both the ESC and the ACC/AHA views.
